Welcome new patients!

Please be sure to download and print the new patient forms before coming into the office. This will prevent any unnecessary delays and ensure prompt patient care.

What to bring to your appointment

  • Insurance Card(s)
  • Valid Driver’s License or Identification Card
  • Referral Form. If your insurance requires a Referral Form from your Primary Care Physician (PCP), it is your responsibility to make sure you bring the hard copy of the form with you. You may also have the PCP’s office fax a copy to our Louetta office at (281) 251-3350. You must make sure that it is in our possession before your visit.
  • Any online forms you filled out prior to your appointment
  • Recent medical records or x-rays will be helpful and may save you time and charges if you are seeking a second opinion or transferring your care

Record Release Forms

For the convenience of our current and former patients, we provide the ability to download forms you can use to request your Louetta Foot and Ankle Specialists medical records and x-rays.

Simply click on Download PDF next to the form(s) you need, print and complete the form(s), and mail or fax your request to the appropriate office. Our office addresses and fax numbers are printed on the forms.

Please be advised Louetta Foot and Ankle will be happy to fax your medical records at no charge to your health care provider, but a small fee will be charged for printed or faxed copies for personal use. Please contact our office for additional information.

What is HealthVault?

HealthVault is a trusted place for people to gather, store, use and share health information online. Registering will give you the opportunity to view your patient information, including medication list and patient education. Through HealthVault.com not only can you view information, but you can send our office a secure message containing any questions, concerns, or medication changes.

After having a severe case of Plantar Fasciitis, we tried all procedures and treatments to reduce pain and inflammation.  Nothing worked.  I was told by Dr. Bachmann of a new procedure, a laser treatment (MLS) that was new but very effective.  I tried it and after treatment #1 I already felt an improvement.  I'm on treatment 9 of 10 and can honestly say it has relieved the pain and constant irritation I was experiencing for the past 9 months.  I would recommend this treatment and the excellent service of Dr. Bachmann's team of health care professionals.
